Compression of the left main coronary artery by a giant pulmonary artery aneurysm.

چکیده
We present a 70-year--old man who had a past medical history of dilated idiopathic pulmonary artery, incidentally diagnosed at the age of 23 years. He was asymptomatic until a year ago when he was admitted for an episode of anginal chest pain in the context of paroxysmal atrial fibrillation. At rapid rates, he showed pathological ST segment descent. A coronary angiography showed a slight stenosis of the left main coronary artery (LMCA). At that time, the mean pulmonary artery pressure was 36 mm Hg. After recovering sinus rhythm, the patient became asymptomatic and was discharged with β-blockers. A year later, the patient was admitted with resting angina. The echocardiogram (Figure A) and the computed tomography-angio showed that the aneurysm had increased to 80 mm and compressed the LMCA (Figure B). This finding was confirmed on coronary angiography and intravascular ultrasound that showed a critical stenosis of LMCA (Figure C, insert 1, and online-only Data Supplement Movie I) and normal luminal diameter of anterior descending artery (Figure C, insert 2). In addition, the mean pulmonary artery pressure had increased >60 mm Hg. Because of the high surgical risk posed aneurysm surgery, percutaneous revascularization of LMCA with drug-eluting stent was performed with good final result (Figure D, insert 1, and online-only Data Supplement Movie II). We also initiated specific treatment of pulmonary hypertension (PH) with phosphodiesterase-5 inhibitors and an endothelin receptor antagonist. The patient became asymptomatic and was discharged to reevaluate the surgical indication of aneurysm according to the clinical and PH evolution in the follow-up. One year later, the patient is asymptomatic with no adverse events. Extrinsic compressions of LMCA by a pulmonary artery aneurysm are extremely rare and usually associated with PH. Only case reports and small series have described this entity as a cause of angina and sudden death, and, currently, the appropriate management of these patients remains unknown.

متن کاملLeft main coronary artery and right pulmonary vein compression by a large pulmonary artery aneurysm.
Pulmonary artery aneurysms are uncommon and may be associated with significant morbidity and mortality. The unique combination of left main coronary artery and upper right pulmonary vein compression by a large pulmonary artery aneurysm is reported. Furthermore, the aetiology, clinical manifestations, complications, diagnostic approach, and possible therapeutic interventions are briefly discussed.
متن کاملLeft main coronary artery compression by a dilated main pulmonary artery and left coronary sinus of Valsalva aneurysm in a patient with heritable pulmonary arterial hypertension and FLNA mutation
Left main coronary artery (LMCA) disease due to external compression by a dilated main pulmonary artery (MPA) is an uncommon clinical entity. Here, we describe a 52-year-old woman with pulmonary arterial hypertension (PAH) and anteroseptal old myocardial infarction (OMI). The cause of the OMI was external compression of the LMCA by the dilated MPA and aneurysm of the left coronary sinus of Vals...
متن کاملA Rare Case of Giant Coronary Aneurysm in a 5- Year- Old Child
Coronary aneurysms are typically seen in association with Kawasaki disease and only in rare instances, they are reported to be congenital or idiopathic. Here we report a case of a five year old boy who was found to have an idiopathic right coronary artery aneurysm. He presented with complaints of recurrent respiratory tract infections. His examination and investigations did not reveal any of th...
متن کاملSinus of Valsalva aneurysm causing extrinsic compression of the left main coronary artery.
Manuscript received December 23, 2014; accepted December 31, 2014. Valsalva (Figure 1) causing critical left main coronary artery compression (Figure 2). Computed tomographic angiography revealed similar findings, which led to the diagnosis of crescendo angina secondary to extrinsic compression of the left main coronary from the giant sinus aneurysm (Figure 3). She was emergently taken to the o...
متن کاملCASE REPORT Left main coronary artery and right pulmonary vein compression by a large pulmonary artery aneurysm
P ulmonary artery aneurysms (PAAs) are uncommon. Depending on their size and location, they may lead to complications such as extrinsic compression of the left main coronary artery and the upper right pulmonary vein, as we describe in our case. Other complications are thrombus formation, secondary thromboembolic phenomena, and rupture. Therefore, awareness of this condition and its diagnosis ar...
